The mission of this Embracing Family Caregivers is to assist in diminishing the emotional, financial and physical toll caregiving can have by assessing care needs, identifying services and establishing a working relationship between the service provider, the care receiver, and the family. Recognizing that most persons wish to remain as independent and in their own homes as long as possible, and knowing that by having the appropriate services in place, this can be allowed to happen.

How to Begin:

The first step is an assessment and status evaluation for persons and families needing to establish a care plan for aged or disabled persons once they are outside of acute care settings, facing discharge from acute care settings or in the midst of chronic health conditions and in need to care coordination. This service is not limited to connecting with care services but also educating the family in techniques that build on the strengths of these resources. Basically navigating the Elder Care System.
